Functional renal insufficiency is not renal damage caused by the ACE inhibitor, but the pharmacological effect of the ACE inhibitors under conditions of severely restricted renal blood flow (see ACE inhibitors and kidneys). The increase in potassium is not clinically relevant in uncomplicated essential hypertension. It is about 0.1 to 0.2 mmol/l. A significant increase in potassium values up to hyperkalemia can only occur if hypotension and renal insufficiency are present at the same time, or if therapeutic potassium administration or potassium saver therapy is carried out at the same time. Functional renal insufficiency is primarily possible in high-risk patients (renal artery stenosis).
